A Note From the Trainer....

This page allows you access to information regarding the paperwork required for participation in intercollegiate athletics at Southwestern Christian University. The documents must be printed, completed, and turned in at the time of the designated physical examination date set for the sport that you are competing in. Failure to do so will result in non-participation until all documents are on file. Use the links below to view and print the documents and please have them ready to turn in at the time they are due.
